What makes the cost go up?

Cap plans work like phone plans: the monthly fee is the only charge until the included value is used, then the per-minute rate applies to the rest. A business taking 200 fixed-line minutes a month on Cap39 pays $39, full stop. One taking 500 pays $39 plus the excess at the plan's rates.
The two things that move your bill
Volume. The inclusions above are sized so the per-minute rate only matters in excess. Estimating your monthly inbound minutes is the single most useful number in choosing a plan — over-estimating wastes plan fees on unused inclusions; under-estimating pushes overflow minutes onto per-minute rates.
Mobile mix. Mobile-originated calls bill at meaningfully higher rates than fixed-line calls — on the entry plans, roughly double to triple. A plumbing line that lives on mobiles has a different cost profile from a B2B line answering office phones. Plans with dedicated mobile-minute inclusions exist for exactly this reason. What does not change the bill
The features — routing, overflow, IVR menus, voicemail-to-email, call recording and reporting — come with the number's service rather than as per-feature add-ons. What can add to the monthly figure is the number itself: a standard number connects at ordinary plan rates, while a premium phoneword (a pattern that spells a word) carries its own lease cost, typically from $40 a month.
